Rental Questions:
Have you received assistance in the last 12 months from any other agency for rent or utilities?
If so, who? Do you know what type of funds they used to assist you?
We have Emergency Food & Shelter Program funds which clients may only be assisted with once per year.
Must have the following:
1. Five day notice and eviction notice (Mortgages are ok)
2. Complete lease agreement Must be a lease six months or longer, no month to month Can not assist with tax credits or subsidized housing such as Section 8
3. Photo ID and social security for all adults listed on the lease
4. Proof of identity for children, i.e. social security cards, birth certificates, etc.
5. Proof of income, must be able to pay next months rent independently
6. Proof of expenses
7. Proof of hardship, i.e. bills, payments, etc. Client must pay the late fees in the form of a money order Can assist with move-ins but most have all the paperwork from the prospective landlord (there is a form)

Utility Assistance Questions:
What kind of utility payment are you seeking assistance with?
We have Project Reach which the client may only be assisted with once per year.
Nevada Power bill must be in the eligible person's name (i.e. senior, etc)

Are you a senior citizen, age 60 and over?
Are you disabled and can show proof of medical necessity?

Need to bring:

3. Proof of medical necessity
4. Proof of active duty of spouse

Employment Support Questions:
Work Cards (TAM, Sheriff's card, & Health Cards) Do you have a letter of intent to hire? (must have letter to prove proof of employment, we have forms) Must have a valid ID and proof of social security number, these items are needed to get these cards.
ID (Monday - Friday 8 A.M - 12:00 P.M. only) Birth Certificates (Wednesday only 8 A.M. - 12:00 P.M.)
Do you have copies of any of your previous identification? If so, what do you have?Client needs to have vital ID information such as social security number proof, copy of birth certificate, copy of government issued id.
Bus passes: 30 day Bus pass: Must show proof of employment 24 hr Bus passes: Are given to assist in getting identification, to attend medical appointments and are only given in conjunction with a meeting with the case manager.
